Glen D. Nelson M.D.
Chairman of the Board

Dr. Nelson served as a member on the Board of Directors at Medtronic beginning in 1980. He joined Medtronic as Executive Vice President in 1986 and was elected Vice Chairman in 1988, a position he held until his retirement. Before joining Medtronic, Dr. Nelson practiced surgery from 1969 to 1986, and served as Chairman of the Board and Chief Executive Officer for American MedCenters, Inc. from 1984 to 1986. From 1975 to 1986, he also served as Chairman, President and Chief Executive Officer of Park Nicollet Medical Center, a large multi-specialty group practice in Minneapolis. Dr. Nelson received his Bachelor of Arts Degree from Harvard College in 1959 and his M.D. Degree from the University of Minnesota in 1963. He completed his training in general surgery at Hennepin County General Hospital, Minneapolis, in 1969. Dr. Nelson currently serves on seven corporate boards.


Gary M. Petrucci
Director

Mr. Petrucci is currently a Managing Director of Investments at Piper Jaffray. Mr. Petrucci joined Piper Jaffray in 1968 as an Investment Executive and served on the Piper Jaffray Board of Directors from 1981 to 1995. He presently serves on the Piper Jaffray Venture Board. Mr. Petrucci has earned the Fred Sirianni Award, honoring the top-producing investment executive at U.S. Bancorp Piper Jaffray, 12 years since the award was established 25 years ago. Mr. Petrucci has served on the Board of Directors for the St. Cloud University Foundation, National Urology Board, American Neck and Back, Reliant Technologies, Lettieri's and America's Bank. Mr. Petrucci is a graduate of St. Cloud State University and did his graduate work in Finance at the University of Minnesota. He is also a graduate of the Wharton School of Securities Industry Institute at the University of Pennsylvania.


Brent G. Blackey
Director

Mr. Blackey is the President and Chief Operating Office (COO) of Holiday Companies, a leader in the petroleum and convenience store industry with over $2 billion in revenues. Prior to joining Holiday Companies in 2004, Mr. Blackey was a Senior Partner at the accounting firms of Arthur Anderson LLP and Ernst & Young LLP. Mr. Blackey was instrumental in leading several clients through both initial and secondary public offerings, and played a key role in numerous transactions with clients Target Corporation, Carlson Companies, Polaris Industries and CH Robinson Worldwide, among others.


John Friedman
Director 

Mr. Friedman is the Managing Partner of Easton Capital Investment Group. Prior to founding Easton Capital, Mr. Friedman was the founder and Managing General Partner of Security Pacific Capital Investors, a $200 million private equity fund geared towards expansion financings and recapitalizations. Previous to Security Pacific, he was a Managing Director and Partner at E.M. Warburg, Pincus & Co., Inc. Mr. Friedman also served as a Managing Director of Atrium Capital Corp., an investment firm. He holds a J.D. degree from Yale Law School and a B.A. degree from Yale College. Mr. Friedman currently serves on the Boards of Renovis, Trellis Bioscience, Xoft, Inc., Sanarus, Inc., YM BioSciences and Assistive Technology. He is Co-Chairman of the Cold Spring Harbor President's Council.


Geoffrey O. Hartzler M.D.
Director

Dr. Hartzler gained international prominence as a cardiologist during 23 years of practice in interventional cardiology. He served most recently, from 1980 to 1995, as a consulting cardiologist with the Mid America Heart Institute of St. Luke's Hospital in Kansas City, MO. Dr. Hartzler was the co-founder and director of four medical device companies, including Ventritex Inc. and IntraLuminal Therapeutics, Inc., and served as a consultant and/or director to seven additional companies, some of which were medical device companies. Dr. Hartzler has published numerous articles in medical and scientific journals on various aspects of cardiology, and has been a frequent lecturer on these topics. Dr. Hartzler is regarded as one of the pioneers of interventional cardiology.


Roger J. Howe Ph.D. Director

Over the past 22 years, Dr. Howe has founded four successful start-up ventures in the technology, information systems and medical products sectors. Dr. Howe currently serves as Chairman of the Board and Chief Financial Officer of Reliant Technologies, a medical laser company he co-founded. From 1996 to 2001, Dr. Howe founded and served as Chief Executive Officer for Metrix Communications, Inc., a business-to-business software development company.


Michael J. Kallok Ph.D. Chief Scientific Officer and Director

Dr. Kallok has more than 25 years of experience in the medical device industry and has been CSI's Chief Scientific Officer since February 15, 2007. Previous to that, Dr. Kallok was Chief Executive Officer of CSI from December 2002 through February 15, 2007. Dr. Kallok has a bachelor's degree from the University of Colorado in Aerospace Engineering Sciences, a master's degree from Purdue University in Engineering Sciences, and a Ph.D. from the University of Minnesota in Biomedical Engineering. Prior to joining Cardiovascular Systems, he held positions at Medtronic, Inc., Angeion Corporation, Myocor, Inc. and Boston Scientific Corporation. He is also founder and president of his own consulting business, Medical Device Consulting, Inc.


David L. Martin
Chief Executive Officer and Director

Mr. Martin has been a driver in building shareholder value throughout the last ten years in three companies. Mr. Martin was appointed CSI's Chief Executive Officer in February 2007 and has served on the board since August 15, 2006. Prior to that, Mr. Martin was Chief Operating Officer for FoxHollow Technologies, (IPO 2004) where he directed their commercial plan. Prior to FoxHollow, Mr. Martin was VP of Global Sales for Rita Medical Systems, (IPO 2000) and before that was Director of U.S. Sales for CardioThoracic Systems, (IPO 1996). Mr. Martin has also held sales and sales management positions for Procter & Gamble, Boston Scientific and Guidant Corporation. Mr. Martin currently serves on two boards.


Christy Wyskiel
Director

Ms. Wyskiel serves as a Managing Director for Maverick Capital in the healthcare group, where she focuses on public and private medical device companies. Maverick Capital currently manages over $9 billion in assets. Prior to joining Maverick in 2002, Ms. Wyskiel served as an Equity Analyst at T. Rowe Price where she focused on the medical device, hospital supply and life science industries. Ms. Wyskiel also served as a Healthcare Associate and Analyst in the investment banking department of SG Cowen. Ms. Wyskiel received an M.B.A. from the Stern School of Business (NYU) and a B.A. from Williams College.
